Thrombopénie thrombosante induite par la vaccination contre la COVID-19 : ce n’est pas toujours une VITT !

RÉSUMÉ

Nous rapportons le cas d’un purpura thrombotique thrombocytopénique (PTT) auto-immun (iPTT) survenant au décours d’une vaccination contre la COVID-19 avec évolution favorable sous caplacizumab. Dix jours après une deuxième dose du vaccin BNT162B2, la patiente s’est présentée aux urgences avec une symptomatologie principalement digestive, accompagnée de céphalées. Le bilan biologique a révélé une anémie hémolytique mécanique et une thrombopénie sévère sans insuffisance rénale. Devant la forte présomption clinique de PTT (FRENCH score à 2 et PLASMIC score à 7), des échanges plasmatiques avec un traitement immunosuppresseur par corticoïdes et anti-CD20 (rituximab) ont été initiés. L’activité ADAMTS13 effondrée avec des anticorps anti-ADAMTS13 positifs a permis de poser le diagnostic formel de iPTT. Devant l’absence de normalisation de la numération plaquettaire (NP) sous EP, corticoïdes et rituximab, un traitement par nanobody anti-vWF (caplacizumab) a été débuté à J13 avec une évolution favorable et une normalisation de la NP et de l’activité ADAMTS13. À un an de cet épisode d’iPTT, la patiente a une activité ADAMTS13 normale sans exacerbation ni rechute. La vaccination contre la COVID-19 a permis de réduire significativement la morbi-mortalité induite par l’infection à SARS-CoV2. Néanmoins, depuis le début de la pandémie, différents effets indésirables des vaccins commercialisés ont été rapportés. L’un des principaux, ayant suscité un engouement dans le milieu médical et médiatique, était la thrombopénie thrombosante induite par le vaccin (VITT). Cet engouement pourrait nuire au diagnostic d’autres étiologies responsables de thrombopénies induites par le vaccin. Le PTT est l’une de ces étiologies, et se caractérise par une forte mortalité. Y penser devant une thrombopénie post-vaccin anti-COVID est donc primordial.

ABSTRACT

We report a case of autoimmune thrombotic thrombocytopenic purpura (iTTP) occurring after COVID-19 vaccination with a favorable outcome after treatment by caplacizumab. Ten days after a second dose of BNT162B2 vaccine, the patient presented to the emergency department with mainly digestive symptoms and headache. The biological workup revealed a mechanical hemolytic anemia and severe thrombocytopenia without renal failure. In view of the strong clinical presumption of TTP (FRENCH score 2 and PLASMIC score 7), plasma exchange with an immunosuppressive treatment with corticoids and anti-CD20 (rituximab) was introduced. Collapsed ADAMTS13 activity with positive anti ADAMTS13 antibodies allowed the formal diagnosis of iTTP. In the absence of normalization of platelet count (PC) under PE, corticoids and rituximab, treatment with anti-VwF nanobody (caplacizumab) was introduced at D13 with a favorable evolution and normalization of PC and ADAMTS13 activity. At one year after this episode of iTTP, the patient has normal ADAMTS13 activity without exacerbation or relapse. Vaccination against COVID-19 significantly reduced the morbidity and mortality induced by SARS-CoV2 infection. Nevertheless, since the beginning of the pandemic, various adverse events have been reported with the different vaccines marketed. One of the main ones, which has caused a stir in the medical and media community, was the occurrence of vaccine-induced thrombotic thrombocytopenia (VITT). This hype may hinder the diagnosis of other etiologies responsible for vaccine-induced thrombocytopenia. TTP is one of these etiologies, and is characterized by a high mortality, so it is important to consider it in the setting of post-vaccine anti-COVID thrombocytopenia.
